Background: AUB (Abnormal uterine bleeding) worsens the quality of life in affected females owing to its negative
effects on professional, sexual, emotional, and physical aspects.
Objectives: The present study was conducted to assess the efficacy of tranexamic acid in managing abnormal uterine
bleeding in females.
Methods: In 48 subjects with excessive menstrual bleeding were included. The parameters assessed were the number
of sanitary pads in two cycles, bleeding duration, and blood loss using PBAC ((Pictorial blood loss assessment
charts). The management was done using 500mg tranexamic acid thrice daily in females with >80ml blood loss and
on PBAC score of more than 100 as assessed in two menstrual cycles.
Results: Hemoglobin of <10 gm% was seen in 72.91% (n=35) subjects, 10 gm% in 22.91% (n=11), and >10 gm% in
4.16% (n=2) study subjects which post-operatively changed following treatment to 41.66% (n=20) subjects with Hb
<10, in 45.83% (n=22) subjects with Hb 10, and 12.5% (n=6) study subjects with Hb >10gm% respectively.
Concerning bleeding, it was seen that before treatment mean bleeding was 120±15ml which decreased significantly
after treatment to 60±11ml. Also, PBAC scores pre-treatment were 150±19 which changed post-treatment to 82±14
which was a significant reduction. Concerning the reduction in blood loss following treatment with 500mg oral
tranexamic acid thrice daily from 1st day of bleeding and continued for 5 following days for 3 months of menstrual
cycle, it was seen that more than 50 ml reduction in blood loss was seen in 79.16% (n=38) subjects, whereas, <50 ml
blood loss was seen in 20.83% (n=10) study subjects. On assessing the percentage reduction in blood loss, it was seen
that <35%, 35-40%, 40-45%, 45-50%, 50-55%, and 55-60% blood loss was seen in 12.5% (n=6), 10.41% (n=5),
18.75% (n=9), 41.66% (n=20), 12.5% (n=6), and 4.16% (n=2) study subjects.
Conclusions: The present study concludes that quality of life in subjects with abnormal uterine bleeding is markedly
improved by decreasing social activity impairment and work impairment with significant improvement in overall
well-being.
