Abstract

Background: The pharmacological closure of hemodynamically significant patent ductus arteriosus (hsPDA) in preterm
newborns has been approved for tibuprofen and indomethacin. According to recent research, PCM (paracetamol) can also be
utilized. Recent data from the literature, however, indicates that further research is required to determine their safety and
effectiveness.
Aim: Ibuprofen and paracetamol's effectiveness in treating hemodynamically severe patent ductus arteries in preterm
newborns was the goal of the current investigation.
Methods: Using 2D echo and clinical results, the study evaluated participants for hemodynamically significant patent ductus
arteriosus (hsPDA). 200 participants were divided into PCM (paracetamol) and IBU (ibuprofen) groups for a three-day
training following the final inclusion of the study participants. When necessary, a second round of the same medication was
administered after clinical and echocardiographic evaluation. Adverse events, medication safety, and ductal closure rate were
evaluated as outcomes.
Methods: The ibuprofen and paracetamol groups had closure rates of 75% and 76.25%, respectively, with a noticeably greater
rate of closure in the first course (16.25%) compared to 45%. Ibuprofen and paracetamol had typical closure times of 4 and
4.54 days, respectively. Subjects with low birth weight and those with platelet counts below normal had a greater closure rate
in the paracetamol group. There was no discernible difference between the two groups in terms of safety.
Conclusion: The current study comes to the conclusion that paracetamol is a better option for patients with low platelet counts
and comorbidities and can be administered as the first line of treatment in situations with hemodynamically severe patent
ductus arteriosus in preterm newborns.
